Confirmation Documents

Please read the instructions <How to Confirm> and download the documents listed below in order to confirm your enrollment in SCJ. 
The deadline is April 26, 2019. 
If you have any problems with downloading the document, please contact scj@icu.ac.jp

<How to Confirm>
Please download the following PDF and read "HOW TO CONFIRM SCJ 2019" carefully. 
Then, please fill the "Form A-D" for your confirmation and submit those forms by the deadline.  



The following items should be submitted via e-mail to scj@icu.ac.jp 
Please send the e-mail as soon as possible. DEADLINE is 26 April, 2019. 
  1. Confirmation Form (Form A)
  2. Payment Form and Receipt of Bank Transfer (Form B)
  3. Visa Details Form (Form C) *Visa Applicants only
  4. Consent Form for JICUF (Form D) *Optional

       (3.5 cm x 3 cm, passport quality. Please write your name in block letters on back side of photos. )

Practical Information for Participants

The SCJ office has prepared this booklet to help ease your transition into the ICU Summer Courses in Japanese(SCJ). Please make sure to read it before your arrival as it contains vital information  related to your stay at ICU.

 Important:【Visa Application】
Because of the procedure is on an individual basis so all application processes should be proceeded at your own risk.

1. First of all, please confirm whether you need to apply for a visa for Japan or not, how to apply for a visa and what kind of documents you should submit to the embassy/ consulate . 
  
2. If you require a visa to enter Japan, we can issue the following documents.
     ・Letter of Guarantee (身元(みもと)保証書(ほしょうしょ)) 
     ・Reason for Invitation (招(しょう)へい理由書(りゆうしょ))  
  ・Detailed Schedule in Japan (滞在(たいざい)予定表(よていひょう)) 
  ・Guarantor's certificates of employment (保証人(ほしょうにん)の在職(ざいしょく)証明書(しょうめいしょ))     
          *Guarantor =President of ICU 
  ・SCJ Catalog 
  ・Your acceptance letter

3. The documents we prepare will be processed after we receive your Form and Payment. To give plenty of time to all the procedures, we establish a deadline.
Please download and return Form A-D by email and pay the Tuition fee by April 12th
  *Personal Information Form and Photos should arrive at ICU by post by April 26th.

4. Please note that the preparation for the documents will take a week at least and the delivery time by EMS varies depending on the region. After receiving the documents, follow the instructions from the embassy/ consulate.

注: 授業料とフォームA-Dを受けとってからビザのための書類を作ります。
ビザ申請に使用するICUからの書類が必要な方は、4月12日までに料金を払い、フォームA-Dをメールで送ってください。
パーソナルインフォメーションシートと写真2枚は、4月26日必着で郵送してください。
